The file "SAS_code" gives the SAS code for the main model described in the paper.  
The file "SensitivityAnalysis" gives the SAS code for the simulations that we did to test the sensitivity of our model outcomes with regard to our C/P ratio weights.  
The file "Karen.dat" contain the actual data, which have been cleaned of data that could easily compromise the identities of our study subjects.
