ACCESS TO THE UTAH POPULATION DATABASE: Access to the Utah Population Database (UPDB), which includes family history records, vital records, cancer registry records, driver license records, and other data types, is regulated by the Utah Resource for Genetic and Epidemiologic Research (RGE). RGE was established by Executive Order of the Governor of Utah in 1982, as a “data resource for the collection, storage, study, and dissemination of medical and related information” for “the purpose of reducing morbidity or mortality, or for the purpose of evaluating and improving the quality of hospital and medical care.”  Accordingly, investigators accessing the linked records comprising the UPDB must obtain RGE and IRB approval.  These approvals were obtained for this manuscript.  Making the data available to other interested parties is possible only if they also obtain approval for access. More information on the UPDB can be found at: http://healthcare.utah.edu/huntsmancancerinstitute/research/updb/ Apply for data access at: http://healthcare.utah.edu/huntsmancancerinstitute/research/updb/access.php Limited access to aggregate data in UPDB for research preparation is available through the UPDB Limited Query Tool at: http://healthcare.utah.edu/huntsmancancerinstitute/research/updb/query.php which was developed by the Pedigree and Population Resource and the Research Informatics Resource at Huntsman Cancer Institute. COMPUTER CODE: All analysis conducted using STAT version 10. The computer code file provide key code for four parts of the analysis 1. The Heckman selection model 2. The trajectory group model 3. The GLLAMM ordered logistic regression model 4. Descriptives for specific conditions by trajectory group Most of the data manipulation is not shown. The paper describes how variables were created.
