1) The data are not publicly available due to privacy restrictions.

2) This study was conducted in accordance with the principles of the Declaration of Helsinki (2013) and was approved by the Area Vasta Centro Ethics Committee (code #10189, approval date 05/16/2018 – code #10187, approval date 11/29/2016 – code #18111, approval date 05/20/2021). All data were fully anonymized before analysis and stored in a secure database.

3) All analyses were conducted using Stata Statistical Software: Release 17 (StataCorp LLC, College Station, TX).
